Minimally invasive surgery in the brain saves life of a 28 year old female with tennis ball size tumor

Doctors at Max Hospital Gurugram successfully removed a tennis ball-sized tumour from the head of a 28-year-old young female, who had been persistently complaining of severe headache for over two months. Pertaining to the size and hard to access location of the tumour, doctors made use of the minimally invasive endoscopic transnasal trans-sphenoidal (TNTS) method to access the tumour through her nose. The patient was not only cured completely but her functional and aesthetic outcomes have also been preserved.

While headache seems to be a common complaint attributed to daily life stress, noise pollution, and /or an underlying medical ailment, the persistent headache should seldom be ignored. Since the patient was having severe intermittent headaches for over two months, made a visit to Max Hospital Gurugram. Upon examination, doctors found a tennis ball-sized tumour in the head.

“The non-cancerous tumour was almost the size of a tennis ball and located at the pituitary gland (which is a pea-sized gland at the base of the brain). She was suffering from a condition known as Pituitary Macroadenoma which is caused by mutations in the DNA of the cells in the pituitary gland.
While the patient upon consultation did not show any signs of nasal discharge, giddiness, visual disturbance and she showed no symptoms of hypothyroidism or hyperthyroidism, she was also opinionated by various departments including Ophthalmology, Neurosurgery, and Endocrinology, pre-operatively.” Said Dr Ravinder Gera, Director and Head of Department, ENT, Max Hospital, Gurugram

While the ophthalmological results were not suggestive of any visual field defects, brain MRI was advised that revealed a well-defined sellar (region around pituitary gland) mass lesion, that was extending into the suprasellar region (fluid-filled space between pituitary and hypothalamus gland) causing superior displacement of the optic chiasma (crossing of the two optic nerves below the brain).

“Being a young female, she was worried about the post complications of the surgery, and her quality of life. But with recent technological advancements available, we performed a transnasal trans-sphenoidal (TNTS) excision or surgical removal of pituitary macroadenoma along with the neurosurgery team under aseptic conditions using endoscopic guidance. The tumour was visualised and was removed completely. The patient has improved post-surgery and resumed normal day-to-day functions.” Added Dr Gera

A 40-year-old, young female Airlines Captain (Mrs. Sudha Bengani), detected with early-stage breast cancer successfully underwent breast conservation surgery followed by chemotherapy at Max Hospital Gurugram and is all set for yet another take-off. Post surgically, she is medically fit (cancer-free) and has recently applied for renewal of her flying license, and is ready to pilot again.

In the meantime, while she is waiting to put on her wings, she and her spouse have been operating a cozy café known as Café Nautical mile which is gathering rave reviews over the net. No prize for guessing why a pilot chose to keep her café name as Nautical mile.

Being in such a profession that demands high levels of physical fitness due to the responsibility it carries with, the news of being detected with breast cancer, seemingly force landed her career. Globally, 7% of the population under the age of 40 years is suffering from breast cancer, whereas in India, the rate is double i.e., 15%. And out of which 1% of the patients are male, which put together has the highest count of breast cancer patients from India globally. Apart from being hereditary, many other risk factors like sedentary lifestyle, bingeing on alcohol, smoking, increasing obesity among youngsters, stress and poor dietary intake have been attributed to increase in the incidences of breast cancer among the young Indian women.

“Diagnosing for breast cancer among younger women (<40 years) is difficult as the tissues being denser than older women. And by the time the lump starts to show persistent symptoms, the tumor reaches an advanced stage, where it proliferates aggressively and less likely to responds to treatment. With the recent advancements in the cancer care treatment, mortality and morbidity rates among breast cancer patients have curbed down. Based on the stage and grade of the tumor, the doctor may prescribe combination of treatments including surgery, chemotherapy, Radiation Therapy, Hormonal Therapy or Targeted Medications.” Added Dr Chugh

There is a dire need to create awareness among masses to get most breast cancers detectable at early stages, as most women with breast cancer arrive after metastasis (when the tumor has spread to other body parts). In metastatic or advanced stages of cancer, it isn’t completely curable, and the treatment aims to achieve remission (where the tumor shrinks or disappears).

Doctors reconstruct wind pipe & food pipe to save life of 22-year-old dying man

Doctors at Max Hospital, Vaishali successfully saved the life of 22-year-old Mohammad Irfan who was brutally injured in a road accident with completely split wind pipe and food pipe. Even the minutest delay in intervention would have costed him his life since his oxygen levels were severely low. Conducting a marathon surgery for 6 hours, his trachea (wind pipe) and oesophagus (food pipe) were meticulously reconstructed to give him a new lease of life.

Irfan, who is a resident of Amroha is a healthcare worker, who after completing his emergency room shift in the morning left the hospital around 8:30 AM. While riding back to his home he suddenly got entangled in a thin calibre rope hanging across the street and fell down with excruciating pain in the neck and breathlessness. Passer-by, rushed him to a local hospital where he was provided with first aid. Owing to the severe injury and deteriorating condition, he was referred to Max Hospital Vaishali and was shifted immediately.

Upon arrival at Max Hospital Vaishali, even though the patient was maintaining saturation, but was unable to talk. He underwent an emergency CT scan at the referring hospital which revealed a complete tracheal transection (splitting up of wind pipe into two) with both ends wide apart, which is a very rare injury and life-threatening condition due to complete loss of ventilation.

“Such severe tracheal injuries are usually fatal and many patients never make it to the hospital. But since his split wind pipe formed a pseudo passage of tissue tunnel, maintaining minor ventilation to the lungs, he was lucky to reach the Operating room on time. He was stabilized and taken for surgery at around 9 PM. Since the food pipe was also vertically injured for around 8-10 cm, though blood vessels were intact, the food pipe was repaired first and thereafter the wind pipe was repaired meticulously. The duration of the entire surgery was around 6 hours. The alternate pipes for food and air were also put in simultaneously to let the repair heal. The patient was bought to consciousness and shifted to ICU thereafter. He was shifted to normal room after two days in ICU.” Said Dr Pramoj Jindal, Director – Thoracic Surgery, Max Hospital, Vaishali.

Post operatively the patient was monitored closely and after a week, video bronchoscopy revealed that the repair was fine, and was discharged to home next day.

One of the major reasons that Irfan’s life was saved is due to the improved territorial connectivity that he was able to reach a well-equipped hospital (Max Hospital Vaishali) in a much shorter time.

Successful management requires emergent evacuation of the injured to a trauma care centre for stabilisation and a quick, safe and efficient transfer to a centre where definitive surgical management (tracheal reconstruction in this case) can be undertaken at the earliest. This rare case exemplifies the successful management of airway trauma where each step was carried out in the recommended manner leading to a successful outcome.

“The patient who is recovering well, is under speech therapy and regular training to get his voice back. While looking at the entire situation, the patient may take another 2-3 months to get his voice back and regular swallowing pattern, but something of a very insignificant time, seeing him alive and healthy.” He added

Amid the COVID era, people have been conscious about their overall health and wellness. Yet there is a dire need to raise awareness about preventing lifestyle ailments that contributes a major burden on the healthcare sector in the country.

It is believed that persistent pain beyond 3-6 months period can lead to changes in the nervous system which can be responsible for continuing pain even when the original cause of pain is removed.
