A 42-year-old man hailing from Virar, Palghar District, Maharashtra has been given a second chance at life after a large size tumor. The tumor was extracted from his jaw by a team led by Dr Sheetal Radia Head and neck cancer Surgeon in Wockhardt Hospital, Mira Road. The patient who suffered from right lower jaw pain and swelling was back on track after undergoing composite bite resection with Bilateral neck dissection surgery for tumour removal. He has been discharged from the hospital and can eat, swallow and speak properly now.

Mr. Rajput who is courier man by profession had developed swelling on the right side of his lower jaw and pain, 2 months ago. He consulted local and many big hospitals but nothing helped him subside his pain or swelling. With time, he started having difficulty in chewing food, due to falling of lower and upper jaw teeth also difficulty in open his mouth. Since the patient’s health continued to deteriorate; he was referred to Wockhardt Hospital, Mira Road wherein he was able to get rid of the tumour.

Dr Atul Narayankar, Consultant Medical Oncologist, Wockhardt Hospital, Mira Road, highlighted, “The patient presented with swelling and pain in the lower right jaw for two months. CT scan and biopsy confirmed that he was suffering from stage IV head and neck cancer (that includes cancers in the buccal mucosa throat, lips, mouth, nose, and salivary gland). Tobacco use, heavy alcohol use, and infection with human papillomavirus (HPV) increase the risk of head and neck cancers. The primary growth of the tumor was large and it has spread to the nodes in the neck.”

Dr. Narayankar added, “As the tumor was locally advanced, upfront surgery was difficult. Hence, it was planned to shrink down the tumor with chemotherapy, and the patient was counselled. He received two cycles of chemotherapy (3 drug regimen) before surgery. The patient had an excellent clinical response to chemotherapy cycles and was then operated by Dr. Sheetal Radia and team.”

Enlarge lymph node involved till behind the neck. After completing chemo sessions, in the month of April and May., the patient was scheduled to undergo Composite bite resection with Bilateral neck dissection to remove the tumor. Also, flap reconstruction that was done by Dr. Akshay Deshpande. The size of the tumor was 9.5cm, maxilla 4cm.”

The more than half lower jaw cut with the cancerous lymph nodes in the neck were removed along with the upper alveolus bone, buccal mucosa muscle, and the skin, and it was replaced with the flap from thigh muscle (anterolateral thigh flap). It is a versatile flap that may be used for any defect in the body that requires soft tissue reconstruction with skin.

Dr. Radia said the procedure lasted for 5 hours and he was discharged after 7 days after surgery. He can eat, swallow, and speak properly now. The pain and swelling have disappeared and have got a near-normal face. Head and neck cancers are one of the most common cancers in India due to the extensive use of tobacco products. If a patient diagnosed at an early stage then surgery with proper management (chemotherapy/surgery/Radiotherapy) these patients can be cured and can live a good quality of life. Also, ongoing COVID-19 pandemic has affected cancer patient management to a large extent all over the world. Delaying treatment worsens the outcome of the disease.”

Q1 FY21 Highlights:

Delhi NCR, 11 Aug 2020: As the world grapples with the COVID 19 pandemic, cancer patients, as well as cancer survivors, are facing particularly unique challenges, with a higher risk of complications from the viral disease, the need to manage their illness and mental health challenges affecting them disproportionately, say experts at leading healthcare provider Paras Healthcare.

With the disease as well as the treatment compromising the immunity of cancer patients and survivors, they face a heightened risk of a series of infections, COVID 19 being one of them. The evidence clearly suggests a higher rate of mortality from COVID 19 among people with cancer.

“Cancer patients and survivors are already dealing with a lot of difficulties. The COVID 19 crisis has only made it more challenging for them to manage their illness. A very critical element of disease management in them, therefore, is the need to safeguard them from all sorts of infections. People with compromised immune systems are not just at higher risk of contracting infections including COVID-19 but are also prone to severe complications from them. This translates into the need for additional precautions and care. Healthcare providers must adopt a comprehensive approach to the treatment of cancer patients during these times including ensuring the continuation of highest standards of care, unhindered treatment as well as psychological support,” said Dr. Dharminder Nagar, Managing Director, Paras Healthcare.

Cancer patients and their healthcare providers are facing a series of challenges during these times. Patients are worried about how to navigate their daily care, whether it is safe to continue visiting hospitals for chemotherapy or other sessions. They are also concerned about being de-prioritized as care receivers in the ‘new normal’. For patients whose treating hospitals have been converted into COVID 19 centres, there is a sudden crisis of how to continue receiving treatments.

“As cancer care providers, we have also grappled with several questions. For example, the pros and cons of starting or continuing chemotherapy at such a time because chemotherapy suppresses the immune system and makes patients more vulnerable to infections. On the other hand, stopping or delaying treatment puts cancer patients at greater risk of disease progression. Till now we have tried to ensure that no treatments are delayed or stopped. It is also extremely important to completely isolate the wards for cancer patients so that the risk of infection is minimized. At Paras Healthcare, we have been highly considerate of the needs of cancer patients and survivors. An often overlooked aspect of cancer treatment is mental health management. The mental health of cancer patients takes a major hit even in normal circumstances. However, currently with a precarious disease situation, they are finding it extremely difficult to cope with the anxiousness and heightened risk of infection.

“We clearly see a rise in the incidence of anxiety and depression among cancer patients in the current circumstances. Offering psychological support, counselling, and medication support if needed to deal with mental health deterioration is important in these circumstances,” adds Dr. Rao.

Two studies published recently in The Lancet journal indicate a high rate of mortality among COVID-19 patients who also have cancer. In one of the studies, data from more than 900 COVID-19 and cancer patients from the US, Canada, and Spain showed a 13% mortality rate. Another study of 800 UK patients with COVID-19 and cancer found a 28% mortality. The studies also concluded that male patients with other COVID-19 mortality risk factors, such as older age and additional comorbidities, had an increased risk of death. However, cancer treatments such as chemotherapy did not appear to have an effect on mortality in either study.

Another study conducted in the US found that 10.4% of 1018 patients with cancer and COVID-19 had died between March and April. As compared to this, the mortality rate was 5.8% for each 100 confirmed cases.

While Malaria and Dengue continue to prevail, water-borne diseases like diarrhoea, jaundice, typhoid are raging all over. Most of these cases are seen from crowded pockets of the city, especially with improper hygiene and water drainage facilities. Besides, the consumption of unwashed raw fruits and vegetables and street food increases the risk of getting sick.

Be watchful of these symptoms

· Several cases of stomach flu or viral gastroenteritis crop up wherein the patients suffer from loose stools, cramps abdominal pain, nausea, vomiting for a few days. · Probiotics like curds, yoghurt have an extremely beneficial effect in treating such cases and can be consumed at home.
· Avoid self-medicating with antibiotics as the majority of cases are due to a viral cause and are self-limiting.
· If there is fever, excessive vomiting, mucus, or blood in stools one must immediately consult a physician for further advice. Hospitalization is needed in such situations.

Kids and the elderly are at a higher risk of suffering from such complications of such diseases thereby making it sure they eat and drink right all the time. Sometimes patients who have chronic intestinal problems like IBS or IBD Inflammatory bowel disease may experience worsening of their symptoms, in which case it is essential to discuss with the primary physician.

RITES total standalone revenue has decreased by 33.9% to `366 crore on account of less exports scheduled for the quarter and impact of restrictions & supply chain disruptions due to pandemic. Similarly, operating revenue, excluding other income, stands at `323 crore in Q1FY21 against `519 crore in Q1FY20. EBITDA and PAT stood at `94 crore and `60 crore against `151 crore and `93 crore, respectively, in Q1FY20. After making adjustment for one-time expenditure on CSR of `9.92 crore and donations (PM-CARES Fund) of `7 crore, EBITDA and PAT stand at `111 crore and `75 crore, respectively, with improved margins of 30.4% and 20.6% against 27.3% and 16.8% in Q1FY20, respectively. Margins were sustained as a result of remote working during lockdown, quick resumption of most of the operations and implementation of cost control measures.

Consolidated

RITES total consolidated revenue has decreased to `379 crore as against `573 crore in Q1FY20. Similarly, operating revenue, excluding other income, stands at `335 crore in Q1FY21 as against `538 crore in Q1FY20. EBITDA and PAT stand at `104 crore and `65 crore against `167 crore and `102 crore, respectively, in Q1FY20. After making adjustment for one-time expenditure on CSR of `10.15 crore and donations of `7 crore, EBITDA and PAT stand at `121 crore and `80 crore, respectively, with improved margins of 31.9% and 21.1% against 29.1% and 17.9% in Q1FY20, respectively.

Performance of standalone segments

Consultancy business continued to provide the highest revenue to the company and achieved a revenue of `185 crore, down by 20.7% over Q1FY20. But, the margins in the consultancy segment were maintained at 44.3%. During this period, in addition to securing more than 60 new projects/contracts including the enhanced scope of work, RITES Ltd signed an MOU with Coal India for providing railway infrastructure technical services for 5 years.

Leasing revenue stands at `24 crore in Q1FY21 as against `29 crore in Q1FY20. The decline was due to suspension of work at certain sites, ports etc. during the lockdown.

Exports revenue during the Q1FY21 stands at `0.5 crore against `129 crore in Q1FY20 as no exports were lined up for Q1FY21. The company has entered into cape gauge markets by securing a contract from Mozambique for export of locomotives and coaches, for which supplies are likely to start in H2 FY21. H2FY21 will also see the export of coaches and DEMUs to Sri Lanka.

Turnkey revenue during Q1FY21 stands at `114 crore against `128 crore in Q1FY20 but we were able to sustain the margins of 4.5%. During this period we have been shortlisted for allotment of work by Indian Railways for railway electrification, civil works, signalling & telecommunication etc. on competition basis among PSUs, for which we will be submitting financial bids by mid of August 2020.

Outlook

Commenting on the outlook, Mr. Mehrotra said, “Despite tough times, we continued to focus on the inclusive growth of the company and our stakeholders. The slowdown in economic activity, supply chain disruptions and travel restrictions impacted the business performance in the first quarter of FY21, however, the financial health of the company remains strong. The company is set to regain momentum with further easing of the restrictions. Based on the existing order book of `6157 crore and expecting recovery in the business environment in H2, the company may achieve moderate growth in the financial year 2020-21 with optimism about the long-term business outlook.”

Bhubaneswar Aug 9: Odisha Library Academy (OLA), a professional group of young library and Information science professionals was established on 9th August 2015 on the eve of Librarians Day. Every year OLA celebrates librarian’s day as well as its annual day followed by a conference or seminar. Due to COVID19 pandemic situation, the academy organised its 5th annual day, a celebration of Librarians day and a panel discussion on Potency of Library:

Past, Present and Future virtually. Professor Ashoka Kumar Das, Vice chairman of Odisha Higher Education Council and former Vice Chancellor Utkal University joined as chief guest. Professor Das elaborated the important role of Librarians and library staffs for the career of a student and highlighted the importance of library for an educational institution. He said the quality of an institute looks from its library, so development of libraries and Librarians is more important for the education system. He also suggested to constitute an advisory committee for the development of libraries and Librarians community in Odisha.

Organising secretaryMr. Niranjan Mohapatra started the programme with a welcome address and president of OLA Dr. Banamber Sahoo gave the Presidential address. Senior Professionals and Professors i.e. Prof. P.V. Konnur, Dr. Daulat Jotwani, Dr. M.P. Tapaswi, Prof. R.N. Mishra had joined as panelists in the panel discussion and Dr. D.B. Ramesh moderate the session.

The Programme was concluded with vote of thanks by the Secretary of OLA Dr. Basudev Mohanty. Odisha Library Academy also celebrated the 128th birth anniversary Dr. S R Ranganatha, the father of Library Science, Information Science and Documentation who born on 9th August 1892. He was honoured with Padmashri Award by Government of India in 1957 for his valuable contributions to Library Science and honoured him with a rare title of “National Research Professor. Every year 12th August, the official birthday of Dr. SR Ranganathan) is being celebrated as National Librarian’s Day in India who had spearheaded library development in India.
