Mrs. Sangeeta Kamdar (name changed) a resident of Mira road suddenly was unable to eat food as she had a non-healing tongue ulcer, unable to eat spicy food, and was unable to speak properly/slurred speech. Initially, she dismissed all the symptoms by thinking it may be normal, she took medicines from a local doctor thinking it’s a simple ulcer. Later when the ulcer was not healing after a long time, she came to Wockhardt Hospitals, Mira Road with a biopsy report suggesting cancer (squamous cancer) wherein her tongue was saved.

Dr. Sheetal Radia, Consultant Otorhinolaryngology and Head & Neck Oncosurgery, Wockhardt Hospital, Mira Road said, “On arrival in OPD, the patient was having a non-healing ulcer. A biopsy was conducted to confirm tongue cancer. Tongue cancer is the type of head and neck cancer that starts in the cells of one’s tongue leading to tumors or lesions in the tongue, Stage 3 cancer. The causes were tobacco chewing, cigarette smoking. It spread to the lymph glands of the neck. Dr. Sheetal has treated many patients with tongue cancer. The line of treatment is surgery followed by chemotherapy and radiotherapy. She would have lost her entire tongue if cancer would not have been detected and treated promptly.”

Dr. Sheetal added, “the patient was discharged 6th day after surgery. She followed up 3 times with us. Radiotherapy and chemotherapy are going on. Tobacco chewing, smoking, or consumption of alcohol in any form should be avoided to prevent the tongue and other cancers of the head & neck. Oral hygiene, dental hygiene should be maintained, sharp teeth ill-fitting dentures should be addressed by the dentist as soon as possible.”

Dr. Radia adds, “Reconstruction was done by using forearm flap (Radial forearm flap) [That is skin, muscles, and tissues from the left wrist were used for reconstruction]. After reconstruction, the patient had good fluent speech – near normal. Her cosmoses were maintained as surgery was done from the inside mouth, there were no external scar marks on the face. Her eating, swallowing, speech, opening-closing of mouth all functions were maintained along with cosmoses.”

“I was at home when I noticed non-healing ulcers for a long time around 2 months. Initially, I didn’t take the symptoms seriously and ignored them. Later, I couldn’t eat food as swallowing became difficult and there was a sore throat. I wasn’t able to speak and my family members found it difficult to understand what I wanted to convey. I was petrified after the diagnosis of tongue cancer. I became numb and was in a state of shock for at least an hour. I locked up myself in the room, didn’t speak to anyone, and cried the entire night. I wasn’t aware of tongue cancer at all. There were many questions and doubts in my mind and wanted to just seek an answer for them. I thought what if I won’t be able to speak again? What am I going to do without my tongue? I couldn’t imagine my life without a tongue. Fortunately, I could seek appropriate treatment at Wockhardt Hospitals and thank the doctors for saving my tongue. I can speak normally and eat properly now. Do not give up if you suffer from tongue cancer, it is possible to treat it and live a normal life,” said patient Mrs. Sangeeta Kamdar (name changed).

The tumor was extracted from his jaw by a team led by Dr Sheetal Radia Head and neck cancer Surgeon in Wockhardt Hospital, Mira Road. The patient who suffered from right lower jaw pain and swelling was back on track after undergoing composite bite resection with Bilateral neck dissection surgery for tumour removal. He has been discharged from the hospital and can eat, swallow and speak properly now.

Mr. Rajput who is courier man by profession had developed swelling on the right side of his lower jaw and pain, 2 months ago. He consulted local and many big hospitals but nothing helped him subside his pain or swelling. With time, he started having difficulty in chewing food, due to falling of lower and upper jaw teeth also difficulty in open his mouth. Since the patient’s health continued to deteriorate; he was referred to Wockhardt Hospital, Mira Road wherein he was able to get rid of the tumour.

Dr Atul Narayankar, Consultant Medical Oncologist, Wockhardt Hospital, Mira Road, highlighted, “The patient presented with swelling and pain in the lower right jaw for two months. CT scan and biopsy confirmed that he was suffering from stage IV head and neck cancer (that includes cancers in the buccal mucosa throat, lips, mouth, nose, and salivary gland). Tobacco use, heavy alcohol use, and infection with human papillomavirus (HPV) increase the risk of head and neck cancers. The primary growth of the tumor was large and it has spread to the nodes in the neck.”

Dr. Narayankar added, “As the tumor was locally advanced, upfront surgery was difficult. Hence, it was planned to shrink down the tumor with chemotherapy, and the patient was counselled. He received two cycles of chemotherapy (3 drug regimen) before surgery. The patient had an excellent clinical response to chemotherapy cycles and was then operated by Dr. Sheetal Radia and team.”

Dr. Sheetal Radia, Head and neck cancer Surgeon, Wockhardt Hospital, Mira Road “The patient encountered right lower jaw pain and swelling and ulcer on the right side of mandible so more than half mandible become nacrose up to the condyle and opposite incisor teeth. Enlarge lymph node involved till behind the neck. After completing chemo sessions, in the month of April and May., the patient was scheduled to undergo Composite bite resection with Bilateral neck dissection to remove the tumor. Also, flap reconstruction that was done by Dr. Akshay Deshpande. The size of the tumor was 9.5cm, maxilla 4cm.”

The more than half lower jaw cut with the cancerous lymph nodes in the neck were removed along with the upper alveolus bone, buccal mucosa muscle, and the skin, and it was replaced with the flap from thigh muscle (anterolateral thigh flap). It is a versatile flap that may be used for any defect in the body that requires soft tissue reconstruction with skin.

Dr. Radia said the procedure lasted for 5 hours and he was discharged after 7 days after surgery. He can eat, swallow, and speak properly now. The pain and swelling have disappeared and have got a near-normal face. Head and neck cancers are one of the most common cancers in India due to the extensive use of tobacco products. If a patient diagnosed at an early stage then surgery with proper management (chemotherapy/surgery/Radiotherapy) these patients can be cured and can live a good quality of life. Also, ongoing COVID-19 pandemic has affected cancer patient management to a large extent all over the world. Delaying treatment worsens the outcome of the disease.”

“I wasn’t able to eat, chew, or speak properly due to jaw pain and swelling. It also took a toll on my self-esteem as I would feel embraced about my foul-smelling breath and fistula with a swollen face. I was devastated when I got to know that I am suffering from cancer. But I was fortunate to receive timely treatment at Wockhardt Hospital in COVID mid pandemic. The hospital is strictly following the COVID-19 protocol. Hence, I wasn’t scared to get operated there. I thank doctors for saving my life,” concluded elated patient Rajput.
