Apollo Telehealth Launches Telemedicine-driven Primary Health Centre in Borobeka, Manipur

Manipur, 31st May, 2024: The recent conflicts in Manipur have caused a significant economic downturn and business disruption, leading to a record-breaking 67,000 individuals being displaced due to ethnic conflict, according to a report. Particularly concerning is the displacement of many healthcare professionals, especially in the hill districts affected by the crisis. Recognizing the critical need to ensure continued access to healthcare services in affected areas, Honourable Justice Gita Mittal, Chairperson of the Supreme Court appointed Committee on Relief and Rehabilitation in Manipur, identified the necessity to operationalize several affected health facilities through electronic modes. In line with this strategy, Apollo Telehealth was selected to oversee the reactivation of the Borobeka Primary Health Centre (PHC) under a public private partnership program.

The Borobeka area, characterized by its isolation and challenging road conditions, presents significant obstacles to accessing timely medical care and emergency services. With the nearest referral hospital located at least 90 minutes away in Jiribam, residents face prolonged treatment and exacerbated health concerns due to these challenges. Fortunately, the collaboration with Apollo Telehealth signifies a proactive effort to restore essential healthcare services to communities affected by recent unrest.

The partnership marked a significant milestone with the presence of Honourable Justice Gita Mittal, Chairperson of the SC-appointed Committee for Rehabilitation of Manipur, along with esteemed dignitaries from the Manipur government. This event culminated a journey that began on March 15, 2024, with the signing of the Memorandum of Understanding. Despite facing formidable challenges including recent communal riots, heavy rainfall, limited access, and poor roads, Apollo’s dedicated team worked tirelessly across departments to transform the Borobeka Primary Health Centre (PHC). Their unwavering commitment and efficient approach ensured the successful completion of this vital project.

Apollo Telehealth spearheaded the installation of essential medical and IT infrastructure, complemented by a proficient team of trained staff. Dedicated high-speed internet connectivity was established to facilitate seamless telemedicine services. Over the course of 30 days of rigorous effort, the primary healthcare centre (PHC) underwent a comprehensive renovation to cater to the needs of the local community. Presently, the PHC boasts a skilled workforce including an MBBS doctor, nurses, lab technicians, and MPHWS professionals. It offers specialized teleconsultations encompassing various fields such as OB/GYN, cardiology, orthopaedics, and nephrology, among others. Furthermore, the PHC is fully equipped to execute national health mission programs addressing NCD, ANC/PNC, mental healthcare, and more. Latest intubation techniques facilitate less complications & speedy recovery of patients: ISCCM

Hyderabad: Indian Society of Critical Care Medicine (ISCCM), Hyderabad Chapter and Apollo Hospitals, Hyderabad; conducted a Difficult Airway Workshop at Apollo Health City Campus, Jubilee Hills, today. Mr. Y. Subramanyam, CEO, Central Zone, Apollo Hospitals, inaugurated the Workshop. Dr. K. Subba Reddy, Sr. Consultant and Head of Critical Care, Apollo Hospitals, Jubilee Hills and other Critical Care consultants shared their experiences in handling airway in sick patients during COVID pandemic. Over 100 Critical care and other specialists from across south India attended and learnt the latest techniques in effective endotracheal Intubation to enhance their skills in airway management.

Intubation is a lifesaving skill for all ICU physicians, where an ET (Endotracheal) tube is inserted into trachea before starting ventilator therapy for patients with low oxygen level and difficulty in breathing. The goal of the workshop was to train critical care doctors to perform intubation effectively and stabilize critical patients early, thereby enhancing the overall patient care.

The workshop dispensed knowledge on newer techniques of intubation like video laryngoscopy and fiberoptic bronchoscopy, among others. The advent of these techniques has immensely facilitated easy identification of the path to intubation by critical care specialists. They enhance the possibility to secure airway in the very first attempt without delay and helps to improve oxygen saturation quickly. These techniques significantly reduce complications associated with intubation and facilitate speedy recovery of patients. They are particularly useful in patients with facial fractures, head & neck injuries during accidents and in cancer patients involving oral cavity and needing ventilator therapy. These techniques also minimize direct exposure of patient’s oral secretions to doctor, which is critical during treatment of patients with lung infections like COVID & TB infections.

Apollo Hospitals performs a complex double organ – Heart & Lung Transplantation

Hyderabad: A team of doctors led by renowned Cardiothoracic Surgeon and Padmasri awardee Dr Alla Gopala Krishna Gokhale performed a rare, combined Heart and Lung transplant on 20th July 2021 to save a terminally ill twenty-five-year-old woman, Ms Labdhi Jain. The recipient was unfortunately born with congenital heart disease with a hole in her heart known as ventricular septal defect and an abnormal connection between her great arteries (Aorto-pulmonary window). Over the years, the congenital defect led to further deterioration in her health condition with increased lung pressures and leakage of heart valves. She was surviving on oxygen support at home for the last two-and-half years. Of late, she was admitted at various hospitals several times for heart failure. The transplant team at Apollo Hospitals, Hyderabad, on evaluation found her to be in an advanced stage of heart failure coupled with increased lung pressures. The team prescribed combined Heart and Lung transplantation to save her life. She was registered under the State Government’s Jeevandan Cadaver Transplantation program and was waiting for a suitable donor.

The team faced unique challenges in this case. The recipient at the time of listing for transplant weighed only 30 kgs. Getting a suitable organ which was of appropriate in size for her was complex, as her heart cavity was small. She was helplessly waiting for a suitable organ for a prolonged period of over two and half years, with hope of finding a small sized organ diminishing by the day.

The cadaver heart and lung obtained from an eighteen-year-old girl, pronounced brain dead due to intracranial bleeding and thrombosis, aptly suited her requirement. Despite the immense sorrow of losing their child, the donor’s family displayed exemplary courage and benevolence and gave consent to harvest their braindead daughter’s organs. This humanitarian gesture on behalf of the grief-stricken loved ones brought joy back in the life of another family.

The recipient recovered well after surgery; her weight has improved to 38 kgs. She is being discharged from the hospital, freed from the burden of heart and lung disease, to enjoy life the way she deserves, and to overcome any obstacles that come her way with confident strides and a happy heart.

  1. Thankfully, 99 % of our worst fears don’t ever come true and the same applies to Covid too! Stay away from Negative thoughts and keep yourself occupied with your profession or hobbies or family. Friends are not the best people to hang around, now!
  1. Exercising regularly conditions the Body and Lungs too! Obesity may harm, not just due to increased risk of complications and even death if one contracts Covid, but also due to Heart attacks, Brain strokes too, to mention a few.  Regular Breathing Exercises like Pranayama do prepare the Lungs and Body, to face the toughest challenges!
  1. Uncontrolled High Blood Pressure and untreated Heart problems aren’t safe either, for the same reasons!

  1. We are what we eat. Eat a healthy balanced diet rich in fresh fruits and vegetables, nuts. Sorry, but No proven value of any unduly high intake of ‘Herbal remedies’! ‘Too much’ intake of even otherwise safe-appearing Herbs and Natural Remedies may harm the Body, including Kidneys and the Liver. Up to you if you want to take the undue risk! Restrict to fair and normal usage only, not in industrial quantities!
  1. Low Vitamin D may adversely affect immunity. If you haven’t tested, no point queuing at labs now. Don’t go out in the scorching Summer heat either, unless you want to invite a Heat Stroke! If you haven’t had Kidney stones or recent Vitamin D supplements and don’t suffer from certain diseases, it may be ok to take some Vitamin D supplements. Speak to your Doctor (not urgent!).
  1. A Cocktail of Multivitamin supplements and minerals has no proven value in boosting Immunity in healthy individuals taking a balanced healthy diet. These are much hyped and oversold! Very few people need these and your doctor may guide you.
  1. A seemingly fun sport, namely Alcohol intake, is known to damage the body, including reducing IMMUNITY. Stay away!
  1. Most importantly, take Covid Vaccine, the proven weapon to prevent Covid, unless if you have contraindications. You may see my other post regarding this. Even if you contract Covid after appropriate Vaccination, Vaccine decreases the risk of severity, hospitalization, and death!
  1. Sleep well, on time, at least 6 to 8 hours daily. Good quality Sleep boosts good health. There are no Magic Pills that instantaneously boost Immunity. Make it a habit to follow these healthy habits during All times and Seasons and Stay Safe and Healthy!

Rare and complex Combined Liver and Kidney Transplant performed at Apollo Hospitals!

Hyderabad, 13th April 2021: Apollo Hospitals, Hyderabad, performed a Complex Combined Liver and Kidney Transplant, using organs from two different living donors, for the first time in India. The Complex surgery was performed by Dr Manish Varma, Chief Transplant Surgeon; by Dr Ravi Andrews, Consultant Nephrologist and Dr Naveen Polavarapu, Chief Transplant Hepatologist at Apollo Hospitals, Hyderabad, on a fifty-seven-year-old Tanzanian patient, Gabriel Ceaser Sisa. The uniqueness of the case was further compounded by the usage of a blood group mismatched organ, the liver. That made the task extremely perilous, but the hands-on experience and the immense expertise, the Transplant and Support Teams at Apollo Hospitals command, steered the outcome to success. The patient has since recovered and is all set to be discharged.

This case is unique, as it is the first time in our country that a multi-organ transplant has been performed using one organ that is mismatched blood group and the other organ of a matching blood group. Monitoring such patients, especially for rejection is a big challenge. As it is, blood group mismatched transplant of a single organ is a complex endeavor, undertaken only in select centers globally. It is noteworthy that the same team had performed the first mismatched blood group transplant of the city about six months ago.

Gabriel Ceaser Sisa first visited Dr Naveen Polavarapu, Apollo Hospitals, Hyderabad in 2017 with Liver related problems. It soon transpired that he had chronic Liver and chronic Kidney disease. Over the course of the next three years his Liver and Kidney diseases progressed despite being on active medical treatment and close monitoring. As he reached end stage Liver disease and end stage Kidney disease requiring hemodialysis, he was advised to undergo a combined liver and kidney transplant. Usually, such transplants are performed using organs from a single cadaveric donor, which avoids complications associated with multiple living donors including blood group mismatch, post-surgery donor recovery etc. In fact, rejection of the blood group mismatched organ by the recipient is very high and is an immense challenge, needing constant monitoring.

This patient being a foreign national was ineligible for a cadaveric organ in India and only Indian citizens can avail cadaver organs as per the law. The doctors had no choice but to harvest organs from two different donors from the patient’s family. Though his brother and nephew accompanied him as possible blood group matching donors for liver and kidney respectively, the brother was found medically unfit to donate either liver or kidney. The search mounted for a same blood group liver donor turned out to be futile. It was his fifty-two-year-old wife with a different blood group was the option on hand and a slice of liver was sourced from her and the twenty-nine-year-old nephew donated kidney.

He was admitted on February 9th, 2021, though preparations, administering special medicines and conducting various procedures as a part of the treatment protocol was begun three weeks ahead of surgery. The surgery, done on February 12th, 2021, took 23 hours, with three teams of surgeons operating simultaneously in three operation theatres. The kidney donor was discharged on the fourth day, liver donor on the sixth day and the recipient after sixteen days after the surgery on February 28th, 2021. The patient and both the donors are healthy and ready to fly back home.

The challenges faced by the medical team were enormous because of multiple health and procedural complications. The patient had to be prepared for blood group mismatched transplant, which required special treatment protocol. This surgery itself was a high risk because of multiple reasons like blood group mismatch, patient on dialysis, dual organ transplant and both organs from living donors, says Dr. Manish Varma, Chief Transplant Surgeon, Apollo Hospitals, Hyderabad.

The expert team of doctors was supported by Dr Ravi Andrews, Consultant Nephrologist; Dr K. Soma Sekhar, Consultant Gastroenterologist; Consultant Transplant Surgeons Dr Sasidhar Reddy and Dr Rajshri; Dr Manjunath B, Consultant Anesthetist and Dr Navakanth, Consultant Intensivist, among others.

This patient has to be on regular and lifelong medicines and undergo tests at regular intervals to constantly monitor both the organs. He needs to take additional precautions from the COVID perspective too.

“This is a very rare and unique case of it’s kind particularly because of the blood group mismatch and the complex combined Liver and kidney transplant done successfully on the same patient. Even on thorough literature search by our medical team only one such case was done earlier in the world. This required meticulous planning and execution and it wouldn’t have been possible without the efforts of our multidisciplinary team. We are happy to see that the patient is going back to his country safely after a successful outcome” says Dr Naveen Polavarapu, Chief Transplant Hepatologist, Apollo Hospitals, Hyderabad.

Hyderabad: The Heart transplant performed on a critical cardiac patient at Apollo Hospitals, Jubilee Hills, with the cadaver heart transported through Metro rail, was successful with the heart recipient recovering fully. For the first time in the history of organ transplantation, anywhere in the world, metro rail was used to transport the organ. A green corridor was specially created by Hyderabad Metro Rail and Hyderabad Police, for the cadaver heart to reach Apollo Hospitals at Jubilee Hills, expeditiously.

This was announced by Dr K Hari Prasad, President, Apollo Group Hospitals;; Dr. Alla Gopala Krishna Gokhale, Sr. Consultant, Heart and Lung Transplant Surgeon, Apollo Hospitals; Mr. Anil Kumar, IPS, Addl. Commissioner of Police, (Traffic) – Hyderabad; Mr. NVS Reddy, Managing Director, HMRL and Mr. Anil Kumar Saini, COO, L&T Metro Rail; at a press conference at Apollo Hospitals. The heart recipient Mr. Singareddy Kanakareddy was also present at the press conference. The 44-year-old recipient was suffering from dilated cardiomyopathy, he desperately needed a heart transplant to survive and was kept alive for over 45 days with continuous infusion of medicine, till he got transplanted with a new heart.

The speedy transportation of the heart by a dedicated metro train during peak traffic hour played a critical role, it enabled to bring the cadaver heart intact and aided in the overall success of the surgery, says Dr. Alla Gopala Krishna Gokhale. Transplantation has now become common, but one interesting aspect is that the Apollo Hospitals Group ranks number two in terms of number of transplants done by a hospital group in the world. This time we created a new green channel of bringing the cadaver heart by Metro Rail, instead of the usual ambulance, helicopter etc., because the situation demanded and by road from one part of the city to another part would have taken lot of time. For a heart transplant we can bring the heart with in 4 hours and transplant, but this patient was critical, he was on a death bed and needed the heart to be transplanted within 1 to 2 hours to get best results. With the help of Police and Metro Rail we brought the organ by a dedicated metro train for the first time in the world. It was a spontaneous thought of exploring the metro option to speed up the transportation which helped. Even Metro Rail responded in less than 10 minutes to support the cause. We were not sure how the patient will recover after heart transplant, but he recovered very well the very next day, was able to walk the third day and getting discharged today. This effort of bringing by metro rail and the excellent coordination between Hospital, Metro rail and Police has to be recorded in transplantation history.

Dr K Hari Prasad said, the patient’s two young children and family would have been on road but for the organ transplant and we need to express gratitude to the donor and his family for coming forward to donate despite their distress, than the Doctors of Kamineni Hospital for counselling the donor family and convincing them, the third party is the Metro Rail which came forward to transport by a dedicated train to reduce the time, the city police for creating a dedicated green channel and being humanitarian to ensure for organ transportation. Jeevandatha the nodal agency managing organ donation also need to be appreciated, because they are the most efficient nodal agency in the country. Dr Gokhale and his team deserve appreciation for the marathon transplantation effort.

Mr. Anil Kumar said, Hyderabad Traffic Police has been creating green channel for organ transportation for the last 2 to 3 years, in fact last year we created the green channel 25 times, almost every week we are doing it this year. We consider it a privilege, it is our way to show that we care for the common man and the public and therefore laying lot of emphasis on such support, we are available 24/7 to extend such support. This event was historical and as Hyderabad’s we feel proud for doing this in our city and saving a precious life.

Mr. Anil kumar Saini said, it’s a great occasion that a life has been saved through a dedicated train. Metro Rail which is the life line of the city and proves it can play a critical role in saving lives, we are ready to serve in future too.

Earlier on February 2nd, 2021; the team of specialists led by Dr. Alla Gopala Krishna Gokhale, harvested the heart of a brain dead 45-year-old male farmer at Kamineni Hospital, LB Nagar. The organ was immediately transported through a road and rail green corridor created specifically for it. The transit which took just about 40 minutes involved transportation of the organ by road from Kamineni Hospital, LB Nagar to Nagole Metro Station, by a dedicated Metro train from Nagole to Jubilee Hills Metro station and later taken by road to Apollo Hospitals, Jubilee Hills. Transporting the organ by road alone would have taken over two hours, especially during the peak traffic hour and could have led to complications in transplanting the organ. The surgery performed to transplant the heart took over eight hours and the patient gradually recovered over the last fortnight. He is hail and healthy and lead a normal life with certain precautions.

Hyderabad: The Republic Day celebrations at Apollo Hospitals, was very special and momentous this year with the Frontline Health Workers who stood firmly against all odds in the service of humanity and bravely combatted the unprecedented COVID 19 challenges, hoisting the tricolour. The team comprised of representatives of Doctors, Nurses, House Keeping staff, Physiotherapists, Dieticians, Ambulance drivers, Lab technicians and Administrative staff. They hoisted the flag on behalf of all those silent workers who went about with the call of duty despite the enormous risks to themselves and their families and relentlessly and selflessly served the patients in their hour of despair.

As a tradition at Apollo Hospitals, Hyderabad, the honour of hoisting the tricolour is reserved for senior Doctors. This year we deviated and presented the honour to Frontline healthcare workers. Patients of Urinary incontinence suffer in silence as it is perceived to be a matter of shame, needs awareness!

International Continence Society (ICS) is promoting awareness regarding urinary incontinence, as this condition is poorly understood, and most patients suffer in silence and consider it to be a cause for shame. The ICS World Continence Week (WCW) is an annual initiative with the primary aim to raise awareness about incontinence related issues. WCW was initiated at ICS 2008 in Cairo with the first-ever World Continence Day.

Urinary incontinence is common. Population studies show a prevalence of 10-42% in Indian women. There is a clear trend toward an increase in prevalence as well as severity with age (5.6% in the 3rd decade to 39% in the 7th decade). Incontinence is less common in men and there are no population studies from India. International studies show a prevalence of 3-5% in young men increasing to 11-34% in elderly men. Men may also develop urinary incontinence as a consequence of radical prostatectomy surgery for cancer.

Incontinence has a profound impact on the quality of life, while incontinence is usually not a health hazard, there is an association with depression and social withdrawal. More importantly, such patients are more likely to suffer from falls (presumably when they rush to the toilet) that may become a sentinel event resulting in fracture and mortality.

Most patients suffer in silence. Over 2/3rd of women fail to seek help even in the West. Studies from India show that embarrassment is perhaps more of an impediment than financial constraint. The situation is probably worse in India.

There are several causes. The process of childbirth stresses the pelvic muscles and support structures and can predispose to incontinence. Other causes could be age-related changes in the bladder or outlet, neurological disease such as stroke, multiple sclerosis or Parkinsonism, diabetes and dementia. It can be aggravated by constipation, concomitant medication or obesity.

Lifestyle changes can help. Reduction of fluid intake and reducing the quantity of coffee, tea and alcohol can reduce symptom severity. The Urological Society of India guidelines also recommends counselling regarding weight loss and avoidance of constipation.

Conservative treatments such as pelvic floor muscle training to strengthen the pelvic floor can improve incontinence. However, many women are unable to contract the pelvic floor at all, or worse, contract the wrong set of muscles. It’s important to start training under supervision. The continence nurse can teach the correct technique as well as use special devices such as the Urostym, a state-of-the-art device that can help women suffering from incontinence, both stress as well as urgency incontinence. Medication such as antimuscarinics and beta-3 agonists can help with urgency incontinence.

Advanced treatment for urgency and urgency incontinence. For those who fail to respond to medication, botulinum toxin bladder injections or sacral neuromodulation can be effective. These therapies are expensive and need an invasive procedure but for those who have severe symptoms, there can be dramatic improvements in the quality of life. Botulinum toxin typically works for about 9-10months while the battery of the neuromodulation device needs to be replaced after about 5 years.

Surgery for stress urinary incontinence. Women with severe stress incontinence can benefit from stress incontinence surgery. This includes a variety of surgical options. Minimally invasive synthetic tapes continue to be effective and are recommended by the Indian guidelines. However, a small proportion of women have adverse outcomes and this needs to be discussed.

For those who prefer a native tissue repair without synthetics, autologous pubovaginal sling is the most effective and durable procedure. Careful counselling is recommended. Young women may be better off choosing a natural tissue repair since the consequences of synthetics over the really long term (such as 40 or 50 years) remain uncertain. Weight loss with Green Tea : Does it really work without Workouts and Diet ?

Some who are overweight try anything and everything at their disposal in order to shed those excess pounds; excess Green Tea consumption being one such. Despite the popular belief that Green Tea aids shedding of excess body weight, there is no robust scientific evidence to suggest that Green Tea usage helps reduce excess body weight in meaningful quantities when used by obese individuals.

Despite this, due to ignorance, desperation or as a shortcut to achieving weight loss or due to the advertisements in the internet and media, Green Tea is being consumed in very high quantities by some individuals who are overweight, in order to achieve weight loss. We really do not know if such over consumption of Green Tea several times a day for several weeks and months affects the body adversely in any way, and its effects on the kidney, liver and other organs.

As a popular meme says, all that one can reduce by consuming Green Tea for weight loss indication is a few pounds of Green Tea in their kitchen, but not the excess body weight! Most Green Teas are not all that appealing to the palate, anyway!!

How about trying a healthy, balanced, low caloric diet in conjunction with exercise and other lifestyle changes such as a good sleep routine under appropriate specialist supervision, to achieve those weight loss goals, in a scientific manner and sustain the same in the long run?!
