There is no dearth of Eye Donors. We need to create more awareness: Nitin Gadkari, Union Minister

National Eye Donation Pledge Contest achieves a record number of 3.08 lakh Eye Pledges from across India in just 14 days from Sept 21 to 4th October.

This was disclosed in the Virtual Valedictory Function of National Contest on Eye Donation Pledge held on the eve of World Sight Day Today. Union Minister Nitin Gadkari was the Chief Guest.

Lucknow District stood first in India in mobilising 35, 717 Eye Pledges, Jabalpur District stood second with 30,641 Eyes Pledged.

Speaking on the occasion, Nitin Gadkari said, there is no dearth of Eye Donors. We need to create more awareness. You come and meet us. We will bring MSME, Social Justice and Tribal Welfare Ministries together to spread the reach of Eye Banks in India. If Sri Lanka achieves a remarkable status of exporting Corneas to 27 countries, why can’t India achieve this? Why can’t we adopt their model? he told Saksham, the organisers.

Due to Corona, the eye donations has come down from 60,000 per annum to just 10% I.e 6,500. In this light of background a massive awareness and promotional drive was launched, which resulted in collecting little over three lakh pledges informed Dr Santosh of Saksham.

A contest towards this effect was organised across India. And the NGOs who procured maximum pledges were declared. Where Lucknow and Jabalpur Districts stood in first two positions.

It was an initiative of CAMBA. CAMBA is a project of SAKSHAM.

CAMBA stands for Cornea Andhatv Mukta Bharat Abhiyan (in Hindi). It means Corneal Blindness Free Bharat Movement.

“Cornea Andhatv Mukt Bharat Abhiyan (CAMBA)” is a pan India movement of Saksham to bring down the incidence of Corneal Blindness in India and to motivate millions of Indians to pledge their eyes for this noble cause. Samadrishti, Kshamata Vikas Evam Anusandhan Mandal (SAKSHAM) is a charitable national organization with a major presence in Hyderabad.

Speaking on the occasion Dr. Prashanth Garg from LV Prasad Eye Institute, Hyderabad said, we need two lakh corneas every year on an assumption that 50% of them are not fit for transplantation. We have more than 400 Eye Banks in India. Still, we collect 60,000 corneas annually and 50% of them are not fit for corneal transplantation. Eye Banks have to be more efficient. Sri Lanka exports Corneas to 27 countries. Their PM and their Buddhism religious gurus promote eye donation. In 2018, Sri Lanka has exported 48,000 corneas to 27 countries, he said.

There are 11 lakh blind in the country and their number is on the rise felt many experts, corneal surgeons and eye banks experts.

Three reasons why the Eye Donation Movement is very vital at this juncture:

1. Due to COVID-19, eye donation has dropped to almost 10% of what was being done previously. Another very big issue being faced is lack of storage for longer times. And to store for 10-12 days, the cost has doubled. The biggest problem has been transport and logistics.

2. So, increase in awareness is most essential during COVID-19 do that there is a surge in eye donation activity after the epidemic plateaus. So all the backlog has to be covered, 60,000 eye donation could decrease to 10,000 this year. And we need to enhance the capacity to clear the extra backlog.

3. The biggest reason to start the movement is to increase awareness. Every year around 80 lakh people die but only 30,000 people donate which is less than 1%. The main reason is to build this awareness and also to make them understand the need.

Cancer mangled lower jaw, reconstructed with 3-D virtual planning in a COVID recovered patient, a first in Telangana!

Hyderabad, September 2020: The team of Oncologists at Apollo Cancer Institutes, Hyderabad, performed the first mandible or jawbone reconstruction of a COVID recovered patient with a massive cancer tumour in the lower jaw, using 3-D virtual computer technology, in the Telangana state and probably for the first time in the country. The 45 years old male patient Mr Rajesh Kumar (name changed), hailing from Hyderabad, presented himself with an extensive cancerous growth on the floor of his mouth and a badly mangled mandible or the lower jawbone, at Apollo Cancer Institute on 24th July 2020. He was suffering with it for the past 3 months and delayed taking treatment due to COVID prevalence. He was detected to be COVID positive two weeks prior to reporting to us, at another private cancer centre and was refused surgery there. On investigation at Apollo Hospitals, he was found to be asymptomatic for COVID and the CT chest did not show any COVID related lung changes. He was re-tested after a week, was found to be negative and a surgery was planned on him.

The surgery involved removal of the extensively spread cancer in the floor of the mouth along with the central segment of the mandible and excision of lymph nodes on both sides of his upper neck. The removal of the central arch of the mandible would result in debilitating effects on the overall structure, speech and swallowing, which would decapacitate the patient and life could become traumatic besides impacting his livelihood. To mitigate the functional disruption of the jaw and reduce disfigurement, the team of Specialists headed by Dr Umanath Nayak, Senior Consultant Head & Neck Surgical Oncologist, and aided by Dr B Rahul, Reconstructive Surgeon, Apollo Cancer Institutes, Hyderabad; performed a mandible reconstruction surgery with a vascularized segment of bone harvested from the fibula, using specialized computer software, the 3-D virtual planning, on 3rd August 2020.

The software aided in contouring Fibula, one of the minor bones in the leg, which is straight too, to the shape of the jaw with high precision. CT scans of the affected jaw and the fibula were fed into the computer and acrylic models and surgical guides were prepared which were then used in the operation theatre to provide a precise match. During the surgery after the removal of cancer and adjoining central part of the mandible, the remodelled segment of the fibula was fixed to the remaining portion of the jawbone with titanium plates and screws. The blood supply to the transplanted bone was restored by connecting its artery and vein to blood vessels in the neck using very fine (10-0) nylon sutures under a microscope. The entire procedure lasted around 10 hours. The patient recovered from the surgery with no major complications was discharged with good cosmetic and functional outcome.

The team of specialists confronted multiple challenges in treating this patient, including the advanced stage of cancer, the resulting disruption in form and function of the jaw and the timing of the surgery, considering the COVID status of the patient. Prolonged surgery in COVID patients can often lead to major complications and a 3-week waiting period is generally recommended from the time patient has tested positive or shows the first signs of infection, however for this patient any delay was detrimental due to cancer proliferation, says Dr Umanath Nayak. The redeeming factor which offset all the impediments was the virtual surgical planning, it enabled precise contouring of the anterior mandibular arch and significantly reduced operating time, he adds.

The surgery was performed by a team of specialists comprising of Dr Umanath Nayak, Senior Consultant; Dr Buggaveeti Rahul, Consultant; Dr Abhishek B, Junior Consultant; Dr Ashwin Shastri, Fellow and Dr Ankita K, Fellow; Department of Head & Neck surgical oncology, Apollo Cancer Institutes, Hyderabad.

Then our spouses got involve to match with hospitals, patients, donors etc. We also observed that we must make plasma donation easy and cumbersome as not many donors were venturing out for many reasons some of which include safety, privacy etc.

Many doctors and hospitals have been prescribing plasma rich in antibodies. The patients and doctors are under stress as the number of patients far outnumber the number of donors coming forward to donate plasma. Donors’ fear of loss of privacy, re-infection and the long waiting at Hospitals make them reluctant to come forward to donate. To ensure that the need in society for COVID Convalescent Plasma is met, Rotary Club of Hyderabad Deccan is launching the plasma bank initiative at the Rotary Challa Blood Bank.

The initiative requires patients and donors to register at the www.plasmaforyou.org platform. The Donors will be pre-screened for antibodies and other blood parameters from samples collected at home by our technicians, and eligible donors are then requested to come to the blood bank or transport to the blood bank at a designated time thus eliminating the worries of re-infection and the discomfort of waiting at Hospitals. The process takes just 15 minutes.

The plasma is made available for patients for Rs 10,500/-, for patients in private hospitals who can afford it. This is just the cost of extraction of plasma from a donor. In the case of poor, we will even provide free of cost-shared Sharath Choudhary.

Corona is becoming uncontrollable in five states which have to Improve their testing and Telangana is one amongst them, the Union Minister of State for Home Affairs informed.

Union Government has given free hands to the state. On Union Governments front, several of our facilities such as Railway, ESI are converted into COVID hospitals he shared.

He dismissed news reports purporting Governor criticising the state government. Governor is the constitutional head of the state. She also being doctor herself it was her concern and she made few observations and suggestions. It doesn’t tantamount to criticising. She has every authority to do so, he said and maintained that the relationship with the state government is cordial and reciprocal.
