Manipal Hospital Bhubaneswar Performs Rare Open-Heart Surgery on a Patient with Sickle Cell Anemia

Bhubaneswar, 6th August 2026: A rare and highly challenging open-heart surgery at Manipal Hospital Bhubaneswar has given 32-year-old Ms. Susmita Sethi a new lease on life after doctors successfully treated two serious heart conditions despite the presence of sickle cell anemia, a hereditary blood disorder that significantly increased the risk of surgery. Led by Dr. Jitendra Nath Patnaik, Senior Consultant – Cardiothoracic Vascular Surgery (CTVS), the multidisciplinary team overcame multiple clinical challenges through careful planning, pre-operative preparation and coordinated care.
 
The patient had been living with severe breathlessness for several years. Over time, her condition worsened to the point where she struggled to perform routine daily activities. She developed swelling in her face and feet, frequently woke up at night feeling suffocated and had experienced several episodes of loss of consciousness due to her worsening heart condition. Concerned about her deteriorating health, she sought treatment at Manipal Hospital Bhubaneswar.
 

Manipal Hospital Bhubaneswar Performs Rare Open-Heart Surgery on a Patient with Sickle Cell Anemia

 

 
A comprehensive cardiac evaluation revealed that she had two major structural heart problems. She was born with a hole in her heart, known as an atrial septal defect (ASD), and had also developed severe leakage of her mitral valve. Both conditions had placed excessive strain on her heart, leading to heart failure symptoms and making open-heart surgery essential. The planned procedure involved closing the hole in the heart with a surgical patch and replacing the damaged mitral valve.
 
However, while preparing her for surgery, the medical team identified another serious challenge. Ms. Susmita was also suffering from sickle cell anemia, a genetic disorder in which red blood cells become hard and crescent-shaped instead of their normal biconcave shape. These abnormal cells are fragile and can break down easily during stress, infections or major surgery, triggering a sickle cell crisis that can become life-threatening. A review of her previous medical records confirmed that she had already suffered two such crises in the past, for which she had been treated at Berhampur Medical College, although her heart disease had not been diagnosed at that time.
 
Open-heart surgery in patients with sickle cell anemia is particularly complex because the procedure requires the use of a heart-lung machine, which temporarily performs the function of the heart and lungs while surgeons operate. During cardiopulmonary bypass, blood passes continuously through artificial tubing and pumps. In patients with sickle cell disease, this process can lead to rapid destruction of the abnormal red blood cells, resulting in severe hemolysis and potentially life-threatening complications.
 
Further investigations showed that nearly 69% of the patient’s circulating red blood cells were sickled. Since a much lower percentage of approximately 25% to 30% is considered safer before such surgery, the team decided to first reduce the number of sickled cells. Two days before surgery, Dr. Debi Prasad Acharya from the Department of Transfusion Medicine performed an exchange transfusion, replacing a significant portion of the patient’s sickled red blood cells with healthy donor blood. This critical step substantially reduced the risk of blood cell breakdown during surgery and made the procedure considerably safer.
 
Following meticulous planning by the cardiothoracic surgeons, anesthesiologists, perfusionists, transfusion medicine specialists and critical care team, the surgery was successfully performed. The hole in the heart was closed with a patch, and the damaged mitral valve was replaced. Ms. Susmita recovered well after surgery and continues to do well on regular follow-up. While her heart disease has been completely corrected, she has been referred to the hematology team for long-term management of her sickle cell anemia to reduce the risk of future crises.
 
Commenting on the case, Dr. Jitendra Nath Patnaik, Senior Consultant – Cardiothoracic Vascular Surgery (CTVS), Manipal Hospital Bhubaneswar, said, “This was one of the most challenging cardiac surgeries we have performed because the patient had severe structural heart disease along with sickle cell anemia, which greatly increased the surgical risk. Careful planning, pre-operative exchange transfusion and close coordination between multiple specialties enabled us to safely complete the surgery and give the patient a new lease on life.”
 
This successful case highlights the importance of early diagnosis, multidisciplinary collaboration and individualized treatment planning in managing patients with multiple complex medical conditions. It also demonstrates that inherited disorders such as sickle cell anemia need not prevent patients from undergoing life-saving heart surgery when managed with appropriate expertise and advanced perioperative care.

The Human Side of Robotic Surgery: Tracking Surgeons’ Stress in the Operating Room

Researchers identify various objective physiological signs of stress in surgeons during real operations 

Although robot-assisted surgery (RAS) gives surgeons remarkable precision, little is known about how their stress levels fluctuate during challenging procedures. In a recent study, researchers from Japan monitored surgeons’ brain, muscle, and heart activity during live RASs and linked these physiological signals to the surgeons’ own stress ratings. The team identified objective physiological markers of stressful moments, which could be leveraged to improve surgical training, operating-room design, and surgeon well-being. 

Robot-assisted surgery (RAS) is one of the most advanced tools in modern medicine. These systems give surgeons extreme precision, allowing procedures to be performed through small incisions that minimize both pain and recovery time. Yet even with this sophisticated technology, operations still depend on a human surgeon, who has to stay intensely focused for hours while making medical decisions. With RAS being increasingly adopted worldwide, understanding what it demands of the surgeon is critical. 

 

Though scientists know surgeons experience varying levels of stress during an operation, little is known about how that stress changes from moment to moment. Most research has relied on simulated surgeries or questionnaires completed after the fact, making it difficult to capture the rapid changes that occur in the operating room during a real procedure. In RAS, stress should be understood not only over time but also as a whole-body response involving the cognitive processing system, musculoskeletal system, and autonomic nervous system. Knowledge of when and how stress manifests physiologically in surgeons could help improve surgical training, how operating rooms are designed, and how well physicians are supported at work. 

In a recent study published online in the journal Surgical Endoscopy on June 30, 2026, a research team led by Professor Yoshihiro Shimomura from the Design Research Institute, Chiba University, Japan, including Dr. Kaiqi Wei and Dr. Nanako Nakamura from the Graduate School of Science and Engineering, Chiba University, and Dr. Shinichi Sakamoto from the Graduate School of Medicine, Chiba University, set out to answer this question by monitoring surgeons during live operations. Their paper presents the results of an observational field study in which multiple physiological signals were recorded simultaneously from seven experienced urologists while they performed robot-assisted procedures.  

Each surgeon wore lightweight sensors that recorded brain activity via electroencephalography, shoulder and neck muscle activity via surface electromyography, and heart activity via electrocardiography. Rather than interrupting surgeons’ mid-operation to ask how stressed they felt, the team used video-stimulated recall. Simply put, after surgery, each surgeon watched a recording of their own console view and marked the moments when they felt their stress level changed, rating it on a scale of 0 to 9. This enabled the researchers to match notable physiological patterns to specific moments of stress without disrupting the procedure. 

By statistically analyzing 151 of these stress-rated moments across the seven surgeons, the team found that four physiological signals reliably tracked how stressed surgeons reported feeling: a pattern of brain activity linked to mental arousal increased, muscle tension in the upper shoulder increased, heart rate sped up, and the natural variability between heartbeats decreased. These signals moved together in a way that lined up consistently with how stressed surgeons said they felt, as confirmed through robust statistical analysis. 

These results showcase how unobtrusive physiological monitoring can provide useful information for understanding the challenges faced by surgeons. “Combining surgeon-reported stress annotations with multimodal physiological monitoring is feasible in RAS and may provide a basis for identifying high-demand operative moments during postoperative review,” explains Prof. Shimomura. 

Looking ahead, the researchers also see this approach as a way to better understand the surgical workplace itself, not just surgeons. “Beyond individual case review, surgeon-state measures could serve as an outcome for evaluating the robotic surgery work system, including console ergonomics, communication routines, workload distribution, procedure-specific difficulty, and training stage. This is the most immediate practical application of the present approach,” remarks Prof. Shimomura. 

Future studies involving more hospitals and additional surgical specialties could help determine how these measurements may support human-centered surgical technology and healthier working environments for healthcare professionals. “Designing environments, equipment, and operational methods based on the results of this line of research will help manage doctors’ stress, improve surgical performance, and enhance surgeons’ own well-being. In addition, these findings could be applied to real-time stress relief interventions for surgeons, triggering briefing protocols to enhance surgical safety and supporting stress management for beginners in training using double-console systems,” concludes Prof. Shimomura.
