Why India is Lacking in Medical Device Manufacturing Despite Being One of The Fastest Growing Healthcare Markets

In recent years, India has emerged as one of the fastest growing healthcare markets in the world with most constituents of healthcare showing impressive results including the medical device sector. In fact, the Indian medical devices market is said to be the fourth largest in Asia and features among the top 20 markets globally. Yet, while healthcare as a whole has become one of the largest sectors in terms of both revenue and employment, nearly 80% of MedTech sector sales in the country continue to emanate from imports. To clarify, healthcare roughly comprises hospitals, medical devices, clinical trials, outsourcing, telemedicine, medical tourism, health insurance and medical equipment.

What is really holding back the Indian medical device sector from developing a truly home-grown industry and what can be done to catalyze the growth and development of a well-developed and indeed flourishing indigenous medical device ecosystem?

Absence of a supportive infrastructure and supply chain network

First, although the government has taken several initiatives in this regard in the recent past, for a country with as high a disease burden as India, there is a lack of a sufficient supportive supply chain network, ancillary manufacturing, and relevant infrastructure for making quality medical devices. This is especially true of high-end devices and in particular those that entail expensive technologies and components. For their part, Indian manufacturers are mostly focused on low-end products for local as well as global markets. According to the government’s own Approach Paper to National Medical Device Policy 2022, the high technology end of the medical devices market is near-completely served by foreign multinational companies. On the other hand, domestic players mainly cater to the low-end consumables segment and have a market share of nearly 65 percent. Although it is changing in recent years, most multinationals have production (and R&D) facilities outside India from where they import products for the Indian market.

Inverted duty structure favoring import of finished goods

Second, the import duties on raw materials and components are generally higher than those on finished products. In other words, there is a low entry barrier for finished medical goods and equipment. This naturally incentivizes players in the medical device sector to bring complete standalone products into the country instead of raw materials and inputs that they could possibly use to innovatively manufacture a completely new product by themselves. In the process, many players effectively become traders and not really manufacturers with respect to several medical devices.

The peculiar nature of the domestic market

Third, even as India has a high population and a high disease burden, from the point of view of manufacturers of high-end and technologically-sophisticated medical devices, there is insufficient demand domestically for them to make investments and run full-fledged operations. Whatever demands that is there, it is limited to hospitals and healthcare facilities as well as individuals and families in the metros and big cities in India. In fact, hospitals in non-metro cities are likely to opt for inexpensive products and devices compared to high-end products stemming of affordability issues. Therefore, besides the usual risks associated with the very nature of a nascent business venture, the long-term sustainability within the country has often played on the minds of the manufacturers within the country.

Lack of relevant educational and skilling base

Fourth, although India produces one of the largest numbers of STEM graduates including engineers, there is a clear lack of alignment between biomedical education and engineering disciplines. For the fourth largest medical device manufacturer in Asia, there are only 217 biomedical engineering colleges in the country offering full-fledged courses, according to an education sector-tracking portal.  Then in a survey by Andhra Pradesh Medtech Zone Ltd. (AMTZ), there are less than 20 courses related to biotechnology engineering across India.

Designing devices for different temperature conditions is not easy

Fifth, from the design engineering standpoint, according to experts, it is difficult to make devices and customize them for different international markets with widely different climatic conditions. This becomes even more difficult in India due to the absence of a supportive ecosystem.

Limited transfer of technology to Indian companies

Sixth, while the rules allowing up to 100% investments for both Greenfield and brownfield through automatic route into this sector have certainly brought in increased investments from foreign companies, it remains to be seen how much of that has translated into the transfer of technology and know-how to their Indian counterparts.

The continued absence of a separate regulator for medical devices

Seventh, even as the government has from time to time brought in new rules and laws with an eye on streamlining and upgrading the regulatory framework for medical devices, the segment continues to be bereft of a separate regulator of its own.

Against the backdrop of the aforementioned reasons, what should the authorities do?

First, while identifying high import-dependent devices and technologies and preparing a priority list, the government should encourage a supportive ecosystem for these high-end and sophisticated medical devices. Those Indian companies willing to make an investment in such high-end quality devices must be suitably incentivized through policy and other financial support. It would also entail imposing high import duties on finished products and reducing taxes on input and intermediate materials. Furthermore, the existing PLI schemes must be extended to more device categories. Importantly, the government should keep its focus on incentivizing core technology-based products and innovations and not get taken in by replica and/or reverse-engineered medical device products. Simultaneously, local value chains for components must also be nurtured and allowed to develop.

Second, there should be hard negotiations with foreign companies on investment and consequent transfer of technology, knowhow, and good practices to domestic companies. We must use a large number of highly skilled professionals (who are trainable) and the relatively low labor costs as a bargaining chip during those negotiations.

Third, against the practice of imposing price caps, the government should allow rationalization of trade margins which would pave the way for a level playing field for all players in the value chain eventually benefitting the end consumers too.

Fourth, human resources up killing and knowledge sharing among different stakeholders is the key to a long-term vision for the industry. According to a survey by Andhra Pradesh Medtech Zone (AMTZ), nearly half of the workforce in the medical device sector is unskilled. To address this gap, structured biomedical engineering courses and relevant curricula on biotechnology derived from the latest developments in the West and elsewhere must be available not only at premium institutions but also at second and third-tiered institutions.

Fifth, the government should aggressively focus on creating a vibrant R&D environment in the country while also facilitating industry-academia collaboration. Across all stages of product development, from conception to design to prototyping to validation and testing, research and innovation should receive impetus. New-age healthcare and MedTech start-ups should also be incentivized to join forces with companies specializing in emerging technologies such as Artificial Intelligence (AI), the Internet of Things (IoT), Robotics, and Nanotechnology, among others.

Sixth, the government could consider targeted procurement from local companies for devices and equipment for which there is sufficient local capacity and competition available in the country. This would give a boost to local production as well as innovation. Last year, the Department for Promotion of Industry and Internal Trade (DPIIT), Ministry of Commerce & Industry has notified 135 in-vitro diagnostic medical devices and 19 medical devices for this purpose. This is a promising development.

Seventh, the protection of IP and patents must be rigorously pursued and implemented.

Federal Bank launches the pilot for Instant KCC developed by the Reserve Bank Innovation Hub

In early September 2022, the Reserve Bank of India (RBI) announced the digitalization of Rural Finance in select districts of Tamil Nadu with the Federal Bank as the partner Bank, and with the active cooperation of the State Government.  The pilot of this strategic initiative, developed by the Reserve Bank Innovation Hub, under the guidance of RBI, was launched in Chennai earlier today.  In a function attended by key dignitaries from the Reserve Bank of India, Reserve Bank Innovation Hub, and Tamil Nadu State Government, the Bank officially launched the pilot for Instant Kisan Credit Card (KCC).

The digital loan scheme, the first of its kind in Agriculture lending, will offer a new experience to the farmers when compared to the conventional method of lending in terms of convenience and turn-around time. The platform has built-in capabilities to enable an end-to-end digital and paperless journey.  The platform uses digital capabilities such as eKYC and design, APIs (Application Programming Interfaces) developed by the TN-eGA, and machine learning capabilities. The facility aims at providing small value loans to small & marginal farmers and is designed to facilitate efficient credit flow to unserved and underserved rural populations.

Commenting on the launch, Mr. Rajesh Bansal, Chief Executive Officer, Reserve Bank Innovation Hub (RBIH) said “In India, rural credit is closely linked to economic growth. In this context, we at RBIH have developed a pilot project for the end-to-end digitalization of the Kisan Credit Card (KCC) lending, in association with RBI. This project, now being launched in Tamil Nadu by Federal Bank with active state government participation, opens up possibilities in making frictionless credit accessible to our farmers. We look forward to the success of this project, and await its implementation across the country.”

Hyderabad, 20th September 2022: With Non-Communicable diseases like diabetes, hypertension, Cardiovascular, Cancers, thyroid, chronic kidney disease, etc., reaching epidemic levels in the city, more so in the vulnerable sections, whose awareness levels and access to health care are inadequate, a city-based NGO, Helping Hand Foundation (HHF), in collaboration with Basheer & Sarwar Babu Khan Trust (BSBT), has launched a Holistic Wellness program for the less privileged.

The Holistic Wellness program is part of the larger prevention and control of non-communicable diseases (NCDs) being run by the NGO in Urban slums since Jan 2022.

The Holistic Wellness program will have risk assessment as per WHO standards, wherein BMI, visceral fat, subcutaneous fat, and Base metabolic rate (BMR) will be measured to assess the overall health status & disease control in known cases of diabetes, hypertension or both and other chronic diseases.

Once the key health matrix is assessed, counseling will be done by a well-trained and qualified clinical nutritionist cum wellness coach. Customized diet plans and guidance on physical activity will be given to the participants, who are found to have high BMI, obesity along with NCDs.

Regular follow-up and close monitoring along with necessary lab tests will be done periodically to keep a check on key metabolic functions and pre-empt any complications that arise out of chronic diseases. Nutritional supplements like plant-based proteins, and vitamins, will be provided on a cost basis to the participants.

“I have reversed my diabetes after undergoing this program for about 6 months and we are attempting to reach out to the masses but more so to the vulnerable groups, who have poor awareness levels and access to basic health care,” said Mr. Mujtaba Hasan Askari of Helping Hand Foundation.

Screening & Survey Data

In the last three months, the NGO conducted three screening camps at Hasanagar Slum, Chintalmet & Tadbund crossroads, where there is a high concentration of pushcart vendors and daily wage earners.

A total of 750 plus adults above the age of 30 years were randomly screened in the first-of-its-kind roadside camps by a team of doctors, dieticians, nurses, and paramedics.

28% of the sample screened were found to be healthy, 36% were found to be overweight with BMIs > 25.

20% were found to have grade 1 obesity with BMI 28 to 30, 7% with grade 2 obesity with BMI > 30, and 5% obesity grade 3 with BMI > 35.

Out of the 68% overweight & obese, more than half already were found to have morbid and comorbid conditions, mainly diabetes and hypertension alone or in combination, and the other half were at significant risk of NCDs in the future.

32% were having either Hypertension or diabetes, 21% had both diabetes and hypertension, and 8% of the vendors were found to have Denovo, unknown diabetes, or hypertension.

Interestingly about 25% of the above cases, presented with a condition called metabolic syndrome, in which blood sugar, blood pressure, and blood lipids are impaired.

“Best time to be an actor” – Pururava Rao

With the boom of OTT platforms, the demand for good actors has gone miraculously high. So many stars whom we almost forgot, are coming up with their own films, web series, and short films which are rich in content and characters.

Talking about his experience Pururava said, “I’m loving the appreciation I’m getting from the audience. The actor not only acted but also wrote the dialogues for the film. He has previously been seen as a Raw officer in ‘Rashtra Kavach OM’ starring Aditya Roy Kapoor and Sanjana Sanghi. He is also known for Adheen’s (2020) short film, which got nominated for Filmfare awards. “Writing happened to me through theatre and ever since I’ve been doing both simultaneously. Thanks to the boom of OTT platforms so many actors are getting a chance to come on screen. I think it’s the best time to be an actor. Of course, your training as an actor plays a crucial role but opportunities have increased for sure.”

Pururava’s upcoming projects involve a Netflix film titled ‘Plan A Plan B’ and a short film with Raghubir Yadav called ‘Aath Aana’. He is also writing a few scripts currently. But it all started with a very naive incident, Pururava explains “Once a producer asked for my work reel and all I could manage was recordings of my theatre plays. I realized I couldn’t sell the shoe by showing the sole. I missed the role. Nevertheless, I started making films with my friends and the very first film ‘Itwar’ we made won an award for India Film Project among 1600 entries from all over the world, we shot that film on a phone. Since then the journey has been uphill”.
